Problem: handleEvaluateErrorAlerts exists but is never scheduled (audit).
Scope: a scheduled evaluation for new-issue, event-spike, and error-rate thresholds → notify via email/Slack/webhook.
Done when: crossing a threshold actually sends a notification.
