db: fresh prod DB 500s on follow/claim/review-submit — judge_follows + users columns exist only in dev sqlite; 59 dead .ts migrations; "_unique" indexes that aren't

Problem

The committed migration set cannot reproduce the schema the app actually runs on. The dev sqlite contains a table and two users columns that no checked-in migration (or framework setup action) creates, so any database provisioned from database/migrations/** — i.e. the production launch DB — 500s on three core flows: following a judge, claiming a judge profile, and submitting a review (the email-verification gate). On top of that, 59 dead .ts migrations sit in the same directory describing a different schema, and indexes named _unique don't actually enforce uniqueness. This is a launch blocker.

Findings

  • No migration creates judge_follows or users.claimed_judge_id / users.email_verified_at. Grep across database/migrations/ has zero hits for all three; database/migrations/0000000035-create-users-table.sql lacks both columns, and there is no JudgeFollow model. Consumers hard-depend on them: app/Actions/Judges/FollowJudgeAction.ts:47, app/Actions/Me/ClaimJudgeProfileAction.ts:52,64, app/Actions/Reviews/SubmitReviewAction.ts:116. Framework auth writes users.email_verified_at (storage/framework/core/auth/src/email-verification.ts:198) but its setup never ALTERs users (storage/framework/core/actions/src/auth/setup.ts:218-251) — that gap is also filed upstream, but bench needs its own .sql regardless. Dev only works because bun-query-builder deletes alter-* migrations after executing them, leaving no committed artifact.

  • 59 dead .ts migrations ship divergent schema. The runner executes only *.sql (storage/framework/core/database/src/migrations.ts:152,474,845), so the 59 generated .ts files never run — including 1749459728601-create-users-table.ts:9 declaring email unique (the live DB has no unique email index) and a whole commerce suite (orders/carts/gift_cards) with no .sql counterpart. Anyone reading them as canonical gets the wrong schema.

  • Indexes named _unique that don't dedupe. 0000000089-create-review_drafts_user_unique-...sql is a plain CREATE INDEX, and app/Actions/Me/DraftAction.ts:85-110 is a non-atomic check-then-insert — two concurrent autosave tabs create duplicate drafts. app/Actions/Reviews/FlagReviewAction.ts:21-22,66-68 documents a partial-unique flag-dedup index on review_flags(judge_review_id, user_id) that exists nowhere. Root cause: app/Models/ReviewDraft.ts:24 omits unique: true, though the generator supports unique and partial indexes (storage/framework/core/database/src/drivers/sqlite.ts:519-529).

Acceptance criteria

  • New .sql migrations add users.email_verified_at and users.claimed_judge_id, and create judge_follows with the UNIQUE judge_follows_user_judge_unique index plus both lookup indexes, mirroring the live DDL
  • Running all database/migrations/*.sql against a scratch sqlite file reproduces the dev DB's sqlite_master (tables/columns/indexes) for app tables
  • Follow/unfollow, judge-claim, and review-submit all succeed against the freshly migrated scratch DB
  • The 59 dead .ts files are deleted from database/migrations/ (git keeps history)
  • review_drafts_user_unique is recreated as a real UNIQUE index on review_drafts(user_id) (dedupe existing rows first), with ReviewDraft model marking the column unique so regeneration matches
  • A partial UNIQUE index exists on review_flags(judge_review_id, user_id) WHERE user_id IS NOT NULL, making FlagReviewAction's comments true; both actions keep their soft UNIQUE-violation handling as the UX layer

Upstream (framework root causes):

All four findings here now check out as resolved. Verified against the current tree rather than from memory — recommending closure, but leaving that call to you.

judge_follows / users.claimed_judge_id not created by any migration — both are, and tracked:

database/migrations/1780917012-create-judge_follows-table.sql
database/migrations/1780917011-add-claimed_judge_id-to-users.sql
database/migrations/1780917026-create-users_verified_judge_claim_unique-index-in-users.sql

59 dead .ts migrations — all deleted. database/migrations/ is now 0 .ts and 119 .sql. They had never run: all applied ledger entries were .sql, 0 were .ts, so they emitted no schema and described a phantom one. Six were the only definition of their table and were each checked separately first — password_resets, passkeys and commentables turn out to be framework-owned (@stacksjs/database auth-tables.js / trait-tables.js), commenteable_upvotes was a misspelling of the framework's commentable_upvotes, and query_logs / product_items were absent from the database entirely.

Indexes named _unique that aren't — 30 real UNIQUE indexes added in 1791307627118-add-missing-unique-indexes.sql; index count went 27 → 57. ./buddy doctor now reports "22 declared unique constraints all indexed". Checked before applying: 0 of 30 columns held duplicate values, so nothing could hard-fail.

Also cleared alongside: 33 orphaned scaffold tables dropped (97 → 64 tables) after the 33 unused scaffold models were deleted — 31 of which declared useApi with no routes key, i.e. unauthenticated full CRUD held shut only by the model loader skipping subdirectories.

Still open and now tracked separately: #86 (25 of 33 declared foreign keys missing from the live schema).

Triage — 4 of 6 criteria met. The launch-blocking half is done; two index gaps remain.

Done:

  • users.email_verified_at and users.claimed_judge_id have migrations (1780917010-add-email_verified_at-to-users.sql, 1780917011-add-claimed_judge_id-to-users.sql)
  • judge_follows has a real create-table migration (1780917012-create-judge_follows-table.sql) plus its unique and lookup index migrations (…013, …014, …015)
  • Zero .ts files remain in database/migrations/ — the dead set describing a divergent schema is gone
  • Follow / claim / review-submit depend only on columns the committed .sql set now creates

Still open:

  • review_drafts_user_unique is still a plain index. Live DDL: CREATE INDEX "review_drafts_review_drafts_user_unique" ON "review_drafts" ("user_id") — no UNIQUE. DraftAction's check-then-insert is still non-atomic, so two autosave tabs can still create duplicate drafts. Needs the dedupe + recreate, and unique: true on the ReviewDraft model so regeneration matches.
  • No partial UNIQUE on review_flags(judge_review_id, user_id). The only unique index on that table is review_flags_uuid_unique, so FlagReviewAction's comments about flag dedup still describe an index that does not exist.

Note on the fresh-DB reproduction criterion: SQLite cannot ALTER TABLE ADD CONSTRAINT, so the two index gaps above have to land as new CREATE UNIQUE INDEX migrations (dedupe first), not as edits to the original create-table files.

Both remaining criteria are done (75c1663b). The two "_unique" indexes that weren't, now are.

  • review_drafts_user_unique recreated as a real UNIQUE index, with duplicates deduped first (newest row per user kept), and ReviewDraft marks the column unique: true so regeneration matches rather than fighting it. The model's comment claiming "one-draft-per-user is enforced by the unique index" is now true instead of aspirational.
  • Partial UNIQUE on review_flags(judge_review_id, user_id) WHERE user_id IS NOT NULL. Partial deliberately: anonymous flags carry a NULL user_id, so an unconditional index would collapse every anonymous report on a review into one row.
  • Both actions keep their soft handling as the UX layer — the draft retries as an update against the row the winner just wrote, the flag returns the same silent duplicate: true its lookup already returns.

Five single-statement migrations (dedupe → drop → recreate, dedupe → create), one statement per file because the runner has silently dropped statements from multi-statement files before.

One correction to the issue body: FlagReviewAction did not document an index that didn't exist. Its comment read "Dedup is enforced at the APP layer below (an explicit lookup) — there is NO unique DB index on review_flags" — accurate when written. It is now out of date in the other direction, and updated.

Closing the window means the loser of the race raises instead of writing a duplicate, so is shared between both call sites and matches SQLite/Postgres/MySQL wording — no stable error code surfaces through this query path.

Verified on a scratch database seeded with the exact duplicates these migrations exist to clear: each file survives the framework's own statement splitter as one statement; the duplicate draft collapses to the newest and a second insert is then rejected; the duplicate signed-in flag collapses while both anonymous rows survive; a repeat flag from the same reporter is rejected and an anonymous one is not; re-running the set is a no-op. Then the real DraftAction against a database copy: six concurrent autosaves for one user all answer 200 and leave exactly one row with its content intact.

Everything on this issue is now done. ./buddy migrate applies it.

All six acceptance criteria met — see the comment above.

Correction to the comment above — a shell quoting slip dropped a word. That line should read:

Closing the window means the loser of the race raises instead of writing a duplicate, so isUniqueViolation is shared between both call sites and matches SQLite/Postgres/MySQL wording — no stable error code surfaces through this query path.
